Yes. Unacceptable forms of identification are as follows:

Yes.  Your document should be filled out prior to your appointment, exluding your signature.

No.  It is required that you sign the document in the presence of the notary.

Yes. Since all appointments are considered mobile you must schedule your appointment in advance. 

Depending on what your needs are, My Favorite Notary Serivce may be able to provide you with the appropriate form at an additional cost.

No.  Prison requirements are very strict for visitors. Therefore, we elect NOT to extend this service.

You will need to bring your own witnesses to the appointment.  They will need to present their identification, sign the notary book and be thumb printed. 

Yes.  After the notary accepts your identification, your identification will be logged into the notary book, the signer(s) will be administered an oath, the document(s) will be notarized, the signer(s) will sign the notary book and be thumbprinted.
